Please fill out, print, sign, if necessary, and return the items listed below to Rana Itani at the main office by Friday, June 3, 2011.  Please note: ALL forms can be filled out online and then printed and signed. Also, except where indicated, you must fill out a form for each child.

Please note that only students who have been admitted (new) or who have re-enrolled (returning) are qualified to register.

RETURNING Preschool – 8th Grade Students

• Copy of updated immunization record card or printout from the physician’s office (updated immunization to be completed by physician)

NEW Preschool & Pre-K Students

NEW K-8 Students
